Sleep disturbances and daytime restlessness are seen in 50 to 80% of children with severe developmental disorders. Although they severely impair the quality of life of the children and families, they are not well addressed and managed by professionals. This is reflected in the lack of validated assessment tools for sleep disturbance in these patients. Our aim was to develop and validate a sleep disturbance questionnaire for children with complex diseases as a standardized tool would be important for the clinical management of these patients as well as for research. Design. Sleep questionnaire for neurological and other complex conditions (SNAKE, Schlaffragebogen für Kinder mit Neurologischen und Anderen Komplexen Erkrankungen) was based on expert opinion and consultation with parents. Its psychometric quality, factor structure, validity and reliability were measured. Setting. In- and out-patient settings for children. Patients or Participants. 224 children and adolescents with neurologic and other complex diseases. Interventions. N/A Measurements and Results. Confirmative factor analysis showed that SNAKE comprises 5 factors as assumed. Fit indices (CFI = 0.93; TLI = 0.92; RMSEA = 0.053), analysis of item characteristic and convergent validity (coherence with measures of sleep, i.e. sleep efficiency and sleep quality) as well as correlation with Sleep Disturbance Scale for Children (SDSC) are either good, or very good. The distribution characteristics of our reference sample may be used as a scientifically-derived reference as a diagnostic tool. Re-test analysis (n=62) depicted high stability of those five factors and good replication of validity. Conclusions. SNAKE is a valid and reliable tool for the diagnosis of sleep disturbances in children with neurological and other complex diseases. To further evaluate SNAKE with respect to specificity and sensitivity and to define normal values we suggest further application of the test to severely affected paediatric patients.
